Database Update

✏️ We've started using a new database to keep track of our congregation's information! So it just makes sense to update our records at the same time!

❓ Please answer the questions that apply to you and that you feel comfortable sharing with us. Don't feel like you have to answer everything if you don't want to.

⚠️ One form is good for one person. Please fill out an individual form for each member of your household.
